You will be asked to provide current proof of this status. In addition, your 501c3 non-profit must be registered and in good standing with the Oregon secretary of state and up to date on required forms and tax files for both the state of Oregon and the IRS. The information listed on the Oregon Secretary of State website and IRS website should match the information provided on this form.

Are 90% or more of the organization's members or participants residents of the North Clackamas School District (aka in-district)?  You will be asked to provide a Roster proving in-district status.  *
For youth sports only, is your organization primarily recreational (no tryouts) or competitive (classics, all stars, travel team)?  Please list out percentage or number of teams that are recreational and competitive.
What is the total cost per season for your program? Please include all fees paid by participants. This should include, but is not limited to registration fees, tryout fees, uniform fees, dues, tournament fees. If different teams or programs pay different fees, please describe this in detail below.
