Terms & Conditions:

1. All information given must be true prior to submission.
2. Kindly contact our operators or check our schedule before application.
3. Booking date for testing shall be made online.
4. Please print this form and bring along with your samples during submission / testing.
5. Date of sample submission and testing may be amended by our operators and are subjected to availability of the instruments as well as the operator.
6. Price can be referred to on our website at https://mjiit.utm.my/analytical-lab/fees-charges/.
7. All samples will be returned to the applicant once tested, we do not keep or dispose of your samples.
8. We have the right to decline your application or testing if the information given is inaccurate or samples given are suspected to be harmful to operators and instruments.
9. Results will be given after payment is received.
10. Modes of Payment: Electronic Fund Transfer (EFT), JomPAY or Pay2Use Prepaid.

Mondays - Thursdays (09:00am to 04:30pm) for Testing
Fridays for Inspections and Periodic Maintenance
Lunch Hour (1:00pm - 2:00pm) / Fridays (12:15pm - 2:45pm)
Off during Weekends & Public Holidays
