G-501: RBT-1 勝任能力評核預訂日期 (Zoom)
本表只供香港自閉症聯盟主辦RBT-1課程，及已繳付RBT勝任能力評核費用參加者。
如非聯盟主辦RBT-1課程學員，恕未能接納申請RBT勝任能力評核。
進行網上RBT勝任能力評核時，請預備下列事項
(1)  評核時請自行安一位成人或8歲或以上兒童
(2)  預備紙張和筆
(3)  桌上型 / 手提電腦 + ExCel 試算表軟件
(4)  預備3至5張圖示咭 (例如：一般物品、顏色、動物...)
(5)  預備3至5項物品 (例如：玩具、一般物品)
(6)  評核需時約45-60分鐘，以Zoom 網上型式進行。
(7)  請挑選最少3個下列時段，工作人員安排確實時段後，會用電郵回覆確實時段。
(8)  截止日期：16/9/2020 (星期三) 正午 12:00

注意事項：申請更改預訂日期及時間
(1)  預訂日期及時間公佈/通知學員起計24小時內可免費更改預訂日期及時間1次
(2)  超過公佈/通知後24小時，如申請更改預訂日期及時間，需要繳交$200行政費
(3)  如第2次更改預訂日期及時間，需要繳交$200行政費，只限不多於2次更改預訂日期及時間
(4)  預訂日期及時間48小時前不接受更改預訂日期及時間，需要重新申請及繳交$800評核費
(5)  遲到15分鐘以上/缺席/60分鐘內未能完成評核，視作未能通過評核，需要重新申請及繳交$800評核費
