STUDENTS who respond YES, to one or more of the questions will be directed to a set of instructions and contacts. STUDENTS must click the "NLCHI ONLINE ASSESSMENT TOOL FOR 811", and are asked to NOT contact Occupational Health unless they are a STUDENT, and also an EH EMPLOYEE.
